Register with our practice

We are using a new online service called Register with a GP surgery that makes it easy to register with this GP surgery.

Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.

The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.

Paper forms are still available to download and from the surgery if you need one. If you are using a paper form, you will need to bring photo ID to the practice to complete your registration.

Named GP

At The Village Green Surgery, we are introducing a Named GP system to provide more consistent and personalised care for our patients. This means that each patient will have a dedicated GP responsible for overseeing their long-term healthcare needs.

What is a Named GP?

A Named GP is the doctor responsible for managing your overall care. They will:

· Have a better understanding of your medical history.

· Oversee your long-term health and treatment plans.

· Provide more continuity in your care.

Can I Still See Other Doctors?

Yes! While your Named GP will be your main doctor, you can still see other GPs when needed. If your Named GP is unavailable, you can book an appointment with another GP, but only for urgent (acute) issues. Ongoing concerns should be discussed with your Named GP.

How Are Appointments Booked?

When booking an appointment, our reception team will:

1. Confirm your full name and date of birth before offering an appointment.

2. Check your Named GP and book your appointment with them whenever possible.

3. If your Named GP is unavailable, an alternative GP can see you for urgent issues only.

Special Requests

If you have specific preferences, such as requesting a female GP, we will do our best to accommodate your needs.

Care Home Patients

For patients in care homes, the following GPs are assigned:

· Dr. Wood – Named GP for Croftdene.

· Dr. Singh – Named GP for Hadrian House.
