Sick/Fit notes

If you feel you need a Sick/Fit note, please ask the GP during your appointment, call the Pharmacy Team on 0191 295 8514, or submit an e-consult. You can Self-Certify for the first 7 days, please ask your employer for more information.

We may need to arrange an appointment to discuss your request further. Sick notes can be backdated for a few weeks whilst waiting for an appointment, but there is a limit on how far back they can be backdated so please submit your request as soon as possible. Emergency or urgent appointments should not be used to request Sick or Fit notes.

Doctors in the hospital are also able to provide you with sick note, for example due to a hospital admission, after an operation, or in clinic. Please speak to your treating team if you feel you need a Sick Note in these scenarios.
